Care Beyond the Clinic

The Straits Geriatrics Centre provides coordinated home based care and palliative support to help patients transition safely from hospital to home, and to continue receiving appropriate medical and nursing care in a familiar environment.

Care is guided by a geriatrician and delivered on a case by case basis, aligned with individual clinical needs and care goals. This may include medical oversight at home, nursing support, and symptom management, with a focus on comfort, safety, and continuity of care.

When priorities shift towards comfort and quality of life, palliative support is introduced to address symptoms, support daily needs, and help patients and families navigate care decisions with clarity and reassurance.
